Patient Forms

We look forward to meeting you at your first visit. When you arrive, you will be greeted by our friendly front desk team members who will assist you with registration. You will meet with one of our expert dentists to discuss any concerns, as well as your dentistry goals and desires, and then undergo a comprehensive dental exam. One of our doctors will assess your dental condition and ensure you are free of oral cancer and gum disease. If all is well, you can choose to have a professional cleaning with one of our experienced hygienists, or schedule that and any additional treatment for a return visit.
